Please can you tell what disease my bunny is suffering from

Oh my gosh! That looks terrible and painful. Please take your rabbit to a vet immediately.

That looks to me like your rabbit could have myxomatosis, if you are in a country that has this disease(Europe, Australia, New Zealand, South and Central America, west coast of the US), and there's swelling around the eyes and nose(as it appears there might be in the picture). It can be fatal and your rabbit will need to be put on the appropriate rabbit safe antibiotic to help deal with the secondary infections that occur. Your rabbit may also need to be put on anti inflammatory/pain meds and may need to be syringe feed rabbit recovery mix if your rabbit isn't eating very well on it's own. You need to take your rabbit to a knowledgeable rabbit vet immediately. There is information on the illness in these links.

In the picture it looks like there is swelling around the eyes and nose, but if there is no swelling and only fur loss and lesions on the skin, your rabbit could have sarcoptes mange and not myxomatosis. Mange is treated with ivermectin or selamectin(Revolution). Dosing and treatment info are in these links.
