Yes, you should take your rabbit to a knowledgeable rabbit vet. The bumps on the nose indicate a health problem. If you're in a country that has myxomatosis, there is a possibility this can be the cause, and if this is the case, this is an emergency and you need to get your rabbit to a knowledgeable rabbit vet right away.

If you aren't in a country with myxomatosis that affects rabbits, then it's possible your rabbit has an allergic reaction to something, bug bites, or it could be mange mites.

Remember, rabbits are known for hiding any illness or physical problems. By the time we see something, it has typically progressed too far. Right now you are seeing something not right. Best not to "wait it out" or for things to get worse. If it can't be myxo, then.. as @JBun stated:

Then it's likely either mange mites or possibly rabbit syphilis. If at all possible, find a vet qualified and experienced with rabbits(not just a regular cat and dog vet). Rabbits need different treatment than cats and dogs, so only a vet properly experienced with rabbits will know their particular needs. If you don't have a rabbit vet to go to, you need to first check any medication prescribed by the vet, before it is given to your rabbit, to make sure it is safe for rabbits. Some medications used for other animals, can kill rabbits.
